NETGEAR Nighthawk Smart Wi-Fi Router, R6700 - AC1750 Wireless Speed & netTALK Duo with builtin Wifi
What is the right way to set this up? Calls only go out most times, a nettalk reset fixes it for a short time. Nettalk tech support is no help.

What modem/gateway is the router connected to?
I misunderstood and thought your nettalk was your ISP.
Its just a voip provider that connects to the router.
Try going into the router and turning of sip alg.
You can also put the nettalks ip address into the routers DMZ.

We switched to the Grandstream, a Nettalk tech put in the right changes & it still deosn't work 100% of the time.
