Did you insert the drive with pwer on or off? If with power off, try pulling it, waiting a couple minutes, and re-inserting.
You may also find some clues as to what is going on in the log zip package. mdstat.log, lsblk.log, sgdisk.log, and smart_history log would be places to look.
