As a follow-up, there were lots of write errors which resulted in thousands of rollbacks in the data.
This device had been upgraded from 6TB drives to 10TB drives & a few times during the upgrade, the unit locked up. The power supply was on its way out... because the drives were swapped with 10TB from 6TB, there was also device md126, in addition to md127. The logs were pointing to various volume errors. So I'm guessing that one or more of the lock-ups during disk migration damaged the volume & the system was never able to fully recover.
I was going to factory reset the system, but I decided to wipe the volume first - created 6 JBOD volumes & wrote files of various sizes to each of them & rebooted, fully expecting to see one or more drive revert the file sizes back to 0-bytes. Everything looked good & there were no disk or volume errors in the logs. And also, the Apps section (and Featured Apps on home page) now show available apps again - that hadn't been working either, and there were errors in the log about not being able to read the XML for the apps.
So I recreated the RAID-5 volume a week ago & so far, the issue has not returned. So problem solved. Factory reset not required.
