CPU Fan stuck on 4218 rpm

Try unpluging just the blue wire and see what happens. I would expect the fan to come on full blast right away with the PWM line disconnected. If it still ramps up, then either the fan controller or that fan is bad, and it's worth trying another fan. I don't really expcet this to be it, but I don't know what else it could be.

Ok Thanks, I'll give that a go.
I left the side off today and it was a few hours before it went over 1600 all the way to 4218 and now sits at 3515 & 4218 rpm alternating between those two speeds. The temp ranges from 46-52.

What does the temperature do as the NAS ramps up the fan speed? If it's doing nothing, or gets even higher, have you verified that you don't have a clogged CPU heat sink? Or maybe the CPU temp sensor is the issue. While your temperature is nowhere near the danger zone, it's probably high enough that the NAS will try to bring it down by bumping up the fan speed. If each bump has no effect, it'll probably just keep bumping to max.

I've tried the blue wire and yes it starts a full speed.
Heat is clean, I made sure of that first.
I've put the wire back on and it now reads 1406rpm & 38c. Sys reads 1406 & 53c, hards 38-40c. And i've the side back on too.

It took 45 mins to kick in. Watched to see what if anything started and it looks like the file search (usr/lib/tracker/tracker-miner-fs) kicks in about the same time. Disabelled it and the fan speed went back to normal with in about 30 seconds.
I keep file search in the off position.

I can see the file search using enough CPU to elevate the temperture some, but it's not going to recover in 30 sec. So there must be something wrong in the way the file search is implemented. It looks like it could be a case of double use of a system variable, but I don't see one that would be involved. Maybe double use of a database entry?
Since most (all?) current NASes don't have a CPU fan and the legacy systems aren't officially supported, I can see how something like that could go untested. I don't have file search enabled, and it's now clear it's not a hardware issue, so I'm out of answers. Time for one of the Netgear programmers to chime in.

Thought I'd best time it, just in case my quess at 30 seconds was wildly out.
28 seconds and you can hear the fan slowing down and after 2 1/2 minutes it's in audible at 1900rpm.
I then enabled it again. It took 2 minutes to start increasing speed and by 3' 10'' was at 3013, 3' 40'' 3515 and 4' 4'' at 4218.
I then broke for lunch.
And 1/2 hour later I then turned off indexing and at 34 seconds you could hear the fan slowing down, and after 1 1/2 minutes was down to 3013 and settled down to 1917.

Is the pro6-service-action_1.0_amd64 add-on to be used with all post OS 6.8.x firmware version on the PRO 6 in order to have the correct fan behaviour?

My Pro6's (with the 4-pin CPU fan connected to the motherboard) are running 6.8.1 with no fan issues, and I have never even heard of pro6-service-action_1.0_amd64.

OK, so that is only for units that have the three-pin fan connected ot the IO card instead of the 4-pin one connected to the motherboard. I do note that the patch is no longer reachable.

OK. Thanks for your explanation.
Not sure which version I have. I did have fan issues in the past which is why I stayed on OS 6.5.0.
I downloaded a copy yesterday just in case I want to upgrade one day. You're right though that the link no longer works.

Since you are well out of warranty, just remove the side panel with the vent holes and take a look.
