You may be able to get an idea by looking at the database entries. Try this from the ReadyNAS command line:
# sqlite3 /apps/.readydlna/files.db 'select path from details where mime like "audio/%"'

All - made progress. sorry for my delay. my job has occupied me.
With your help I was able to find dulpicates of the actual songs. Some how inside one of the [share]/ itunes/music folders .i.e, {share}/itunes/music/various - itunes/music was duplicated inside it. AND within that it was dupicated again.
Now I'm still chasing a few duplicates but most of them have been deleted and the DNLA clients have clean list (there are a few transient duplicates).
I confirmed from backups that somehow it happend when the data was restored from the NV+ backup on the external USB drive that was used. The USB drive was clean but in the upload this weirdness must have occured.

This can happen if you move/rename folders and then make an incremental backup - the original files stay in tact, and new (renamed) get added. After a restore, you have multiple copies.
This experience highlights the importance of reducing the number of files for testing purposes ;).
Good news it's sorted and not a real scary problem with the RN...
