Can you still get to the files after removing the new disk that you just inserted? Do you have full backup of the data?
When replacing the disk that has gone faulty, it has to be of the same size of the faulty one, unless you are going to expand the volume.
What's the current firmware version of the NAS?

No, removing the new disk has no effect. It does not return to its previous state where it was working perfectly. In fact, I also tried removing all of the disks, just to see if I could get it to do something different, but I keep getting the same result. I reboot by unplugging, plugging then powering up by pressing the power button. The green lights on the front of the unit corresponding to drives #1 and #3 light up, even when there are no drives inside. The lower window on the front of the unit stays dark and now, in addition, I hear a grinding sound, which is a new development. And after booting up, no matter how long I press the power button, it will not power down unless I unplug. Cant get to the files. It doesnt show up on the network.
I thought it was okay to go up in size with the new drive, but I just wouldnt get the additional space until I replaced all of the drives with the same increased capacity. Is that wrong? I'm not sure of the current firmware, but it was last updated about two years ago. Can't get into it to see what firmware I'm currently running.
I dont have any back-up of the data. It is full of CD's I have ripped and I stilll have all of the CD's. So, if I lose the data on the NAS, it will be a pain to rip the entire 2,500 CD collection again, but at least I have the data.

That's the code for "Boot loader fails to locate valid RAIDiator image, TFTP or USB OS image recovery required." You need to do a USB OS recovery. For more LED codes, see here: ReadyNAS LED Language.
Now, why that happened concurrent with replacing a drive is another question. Maybe just coincidence, but how likely is that?

I suggest that if you want to try USB recovery, you should do that with only the new 1 TB disk installed in the NV+ (removing all the old disks, and labeling them by slot). The procedure is here: https://kb.netgear.com/21400/RAIDiator-4-1-USB-Recovery-Tool. That preserves your existing data while you bring the NAS back up.
You might also want to test the new drive with vendor diags in a windows PC (using WD's Lifeguard program or Seagate's Seatools).
Another option is that you can mount the disks in a normal X86 PC running linux (though you do need to connect all the drives to the PC). Most PCs can be booted up on linux "live" disks, leaving the main OS intact. Then you can offload the data. That procedure is here: http://home.bott.ca/webserver/?p=306
