Your symptom sounds like you are trying to boot an OS6 recovery USB. For legacy units updated to OS6, you have to use an OS4.2.x style USB recovery key but re-name the OS6 image file as if it's an OS4.2.x one.

Sorry about the lag - I've been pretty busy.
The disks look ok. But possibly the file system is damaged.
You could check for that with tech support mode, but it does require running several linux commands.
Do you have a backup? If you do, another path is to do a factory reset and restore the data. Time consuming, but ensures you have a completely clean file system.

Sorry, I've only just seen your reply too. I'm happy to run any list of commands you suggest as it be good to see if we can fix this.
I had some backups from a few months ago and personal stuff is all backed up and ok and up-to-date. But have been working on some old synth libraries recently which were not backed up so, will be handy to have those back.
In the meantime, I thought I would look up how to connect external USB storage, mount and use that to transfer files over. That went ok-ish but large files were not copied due to me using fat32 (for compatibility).
So, I decided to try using a network backup alternative with NFS using rsync to my Synology. I have setup the Synology now with a "recovery" shared drive and finally managed to mount that successfully via ReadyNAS shell. I have an initial file copy running over the network ok and nothing has crashed so far touch wood!
If that finishes ok, I've got the rest of the commands ready to run from SecureCRT to copy the rest of the directories.
If it completes ok, I'll run the commands you suggest to see if that fixes things - as would be good to know. If it does and all is stable again, will try firmware upgrade to OS 6 6.10.10 with the 4.2 recovery usb built after Sandshark mentioned I was using the incorrect version if needs be.

Ok. Let me know when you're ready to proceed. This is a bit different from the usual cases, as you do have access to the data volume. So it possible that your issue is actually with the OS. When you get to it, check to see if the OS partition is full.

I did actually mount the data manually just to see if I access the filesystem to ultimately copy it somewhere, so this is probably why it's a bit different.
I've managed to copy all the files from /data (/dev/md127) to the Synology via rsync, so looked up some commands to run to check fs for errors and after dismounting '/dev/md127' ran a "btrfs check /dev/md127" and it returned with some errors:

The whole issue started when I tried to revert to a previous snapshot on one of the shares and the unit crashed thereafter.
