What is the Mfr and model# of the Internet Service Providers modem/ONT the NG router is connected too?

I currently have a N600 WNDR3400 Yep its a tad old & WiFi Ext
House size apprx 2800sqft 3 levels Log Home
Devices: 1ea Laptop-2ea Ipads-2ea Cell-7ea TV all stream w/Youtube TV Live
Retired Couple w/2ea Grandsons that visit & they watch Youtube & some gaming
Providers Web-Site stats no combo unit / We never have had issues with providor
