I found the solution. I use Kaspersky internet security on all of my computers. If I disable kaspersky, everything loads and displays properly.
edit: to resolve without disabling kaspersky entirely, right click on settings, additional, network, then disable "traffic processing: inject script into web traffic to interact with web pages"
Doing this will disable safe money and a few other plugins, but the modem status page will work correctly.

This issue has been fixed in the latest update of Kaspersky, released March 13 2017.
