WNDR4300V2 - No internet or service access when connected to cable modem

I've been using a N750 router with a Motorola SurfBoard cable modem and Comcast internet service for several years.  No changes have been made to my network, but yesterday I noticed all of our wifi devices were no longer connected, nor were any wire-connected devices.  Here is a summary of all the things I have tried and/or discovered:

  • All physical connections are fine
  • All indicator lights are normal
  • The router firmware is up-to-date (1.0.0.58)
  • A factory reset and power-cycling of all hardware changed nothing
  • When the cable modem is disconnected from the router, the router works as expected (i.e. I can access the configuration utility through a browser, WiFi devices connect normally, and the network communicates normally with all connected devices).
  • When the cable modem is connected directly to the PC, internet connectivity is restored and functions normally.
  • When the cable modem is reconnected to the router and everything power-cycled (modem powered on first then the router), internet connectivity returns for several seconds to a minute, then fails.
  • I have reflashed the firmware successfully with no change
