Did you try re-entering the passphrase in the extender's web ui?

It is possible to access the web admin page from the router network, or by connecting a PC to the extender ethernet. Though you could alternatively do a factory reset on the extender.

I've tried to factory reset and tried the WOS several times no luck

Then try again, and set up the extender w/o using WPS. See Connect with the NETGEAR Installation Assistant on page 11 here: http://www.downloads.netgear.com/files/GDC/EX6110/EX6110_UM_EN.pdf
