EX6150v2 resetting to factory defaults

My EX6150v2 is resetting itself to factory defaults. I'm guessing after a powerful outage, but unsure.  It's configured for access point and type ethernet running firmware 1.0.1.108. But when it arbitrary resets, it leaves my entire network exposed as it fails open.

What is causing this, how to prevent it and if it has to reset, reset closed and prevent unauthorized access?

Please explain what "fails open" means.

CrimpOn, for example, a firewall configured to fail closed will block all traffic if it crashes, preventing unauthorized access during the failure. In physical security, a courthouse metal detector failing closed means no one gets through until it's fixed.

When the EX6150v2 reverts back to factory defaults on its own, it should prevent unauthorized access and fail closed as opposed to failing open and allowing anyone within range to access my network (i.e. open Wi-Fi with no encryption.)
