Edit not sure if this will reset the port back to 8086 but worth a try as it only takes 5 minutes to do.
OS reinstall will only reset the network adapters, not the add-in information.
My lab is taking longer than expected, ran into some difficulties -- old firmware on box and such.
Upgrading it to 4.2.21 and then will proceed to work on the issue.
