Close, but DEVPATH doesn't quite fix it. It doesn't follow the device to its new number, it just lets fancontrol notice the number changed. On a boot where the chip comes up as hwmon3 instead of hwmon4, fancontrol refuses to start with "Configuration appears to be outdated, please run pwmconfig again". So you swap a wrong fan mapping for no fan control at all, which is safer but the same annoyance.
If you want to stay with fancontrol, the version that survives reboots is to drop the DEVPATH and DEVNAME lines and point everything at the full /sys/devices/platform/... path instead of hwmonN. That path doesn't change between boots, so it gets you the same result as looking the device up by name.
