Do the other drives get the same Command timeout errors? Are all drives have the same make/model family?
It would be best if you can run a disk test using the Boot menu on the NAS or by plugging in the 12TB drive on your PC and run Seatools or other available disk utility like Acronis.
The 12TB drive might have been replaced but unfortunately it's not always a Brand new replacement, most would be a refurbished/re certified units. It would be better to make sure that the drive does not have any errors.
If the issue still persist, it would be good to turn to check to turn off the NAS, remove the drives and label them accordingly and insert a spare disk on the slot where the 12TB is located. It will initiate a setup using the spare disk, once done we'll be able to check if it will give the same error and the NAS might need a hardware check.

It is only the 12TB drive (on both the original and on the RMA repacement) that is generating the "Detected increasing command timeouts errors".
In full..."Detected increasing command timeouts errors: [14] on disk 2 (Internal) [ST12000VN0008-2JH101, ZHZ1ENC9] 10 times in the past 30 days. This condition often indicates an impending failure. Be prepared to replace this disk to maintain data redundancy."

I don't think it would be a power supply issue where the needed power is not met thus getting Command timeouts on the 12TB. But it just might be a possible cause.
Can you please send in the logs using a download link sent to me via PM.
If possible, kindly try our suggestion of doing a spare disk or switching the disk bays by turning off the NAS first just like what StephenB mentioned. This is to check if the Command timeouts follows the 12TB so we can at least confirm.

Yes, this error is not really symptomatic of any power issues. I suspect there is a firmware issue with the cache flush handling of the 12TB drive in the ReadyNAS 316.
I am running all 12TB drives in a new ReadyNAS 628x without any "increasing command timeouts errors" being reported on any of those drives. My ReadyNAS 316 however, has this one 12TB HDD in Disk / Channel 2 that is reporting increasing command timeouts errors" (reporitng on the original new drive a few weeks after it being inserted and then similarly also reporting on the RMA drive a few weeks after being inserted).
I note the logical suggestion to move the 12TB drive to a different slot and seeing if the problem follows the drive. I am not able to swap two drives without breaking the X-RAID (RAID 5). Also, I currently have no spare 8TB or 12TB HDDD to swap into Disk / Channel 2 slot in place of the current 12TB that Disk / Channel 2 is reporting . I will have to buy one.
If the issue is a firmware problem (or vice versa a disk problem), then simply swapping the dive to a new slot is not really a clear black and white conclusive "binary test" that pin points the drive being at fault. If the fault continues, it could still be firmware I/O handling related.
