The update should work. There are ways to troubleshoot why it's not working using SSH when booted normally or telnet (tech support mode).
If the prep add-on is installed prior to the R4toR6 image it should reboot multiple times and do a factory reset at the appropriate time. If you don't install the prep add on then it should reboot multiple times (I think) and eventually get stuck and you would need to do a factory reset using the boot menu.
Have you checked the output of
# dmesg
shortly after booting to see if it's reporting any problem with the internal flash?

I don't recall the conversion re-booting multiple times. There is the standard re-boot you have to do to initiate the actual update, just as with any other, then the factory default. Without the Prep add-on, it does lock up in the update process after the re-boot. I believe it successfully writes the update to the flash but then cannot do so to the OS volume and hangs there. I don't recall what the displayed message is at that point, but it may just be "Updating".
