First of all, if you have only added credentials to the Windows system, all you need to do is delete them again, and you should be back to where you started.
So try that first - deleting both the windows and generic credentials you added. After that reboot, and see if access comes back.
On the credentials themselves-
(a) you definitely want windows credentials not generic
(b) the IP address and the NASNAME are treated as different machines, so they can be both present (and use different user names and passwords). You don't need to set them both up, but if you access by both IP address and nasname it is a good idea.
