The Public Ports would be the ones used when people remotely connect via the internet and the Private Ports would be the local one used when on the same LAN as the NAS. You want to enter 8086 for all of those. For the Private IP address you want to enter the static IP address of your NAS.
Perhaps "personal web sharing" would be the best fit.
