RAIDar 6.1 is the latest version - but Netgear is more focused on OS6 NAS than older end-of-life NAS. There are some issues with RAIDar 6.x and the legacy NAS.
The simplest answer is to use RAIDar 4.3.8 with your older NAS.

I know. But it still is a duo v1. Ready the link I posted, it explains the labeling. Calling it a v2 can result in getting wrong advice here.

The only issue with RAIDar 6 and RAIDiator 4.1 is that some earlier versions of RAIDiator 4.1 (4.1.0-4.1.3) use an extremely old protocol; for those versions, you would want to use RAIDar 4. RAIDiator 4.1.3+, RAIDiator 4.2, RAIDiator 5.3, and ReadyNAS OS 6 should all work with RAIDar 6.

Well for some legacy models if you choose Flex-RAID using RAIDar 6.x it has ignored the RAID level choice. I don't think that's restricted to very old RAIDiator firmware.
So I would give RAIDar 6.1 a try and if it doesn't do what you want then try RAIDar 4.3.8.

The net is that the OP already did that, and reported that 4.3.8 worked and 6.1 did not. That is why I gave the OP the reply I did.
With all due respect for Kohdee, I think this shows there clearly are some compatibility issues with RAIDar 6 and OS 4 - perhaps not known to Netgear development.
