ReadyNAS 102 - Addition of second drive for redundancy
Originally set up with one WD 4TB disk (green). NAS Firmware version 6.4.2 Recently bought another WD disk (identical model number). Plan to add disk into x-raid for resiliance. Powered down NAS....
The issue turned out to be a DOA disk. I've since RMA'd it and sourced another. Inserted and added new, working HD, to volume without issue.
Perhaps try removing and reinserting the new disk with the NAS running, and see what happens.
Be very careful to remove the correct drive!

It's unfortunate but some disks do arrive D.O.A.
Disks failing at any time is one reason why backups are important. Redundancy is great and I use it but it's no replacement for backups.
Say for example you had one disk in the NAS then added a new disk and the old disk failed mid-way through the resync you'd have a big problem unless you have a backup.
