Posting here to share the experience with others. As discussed 3 out of 4 disks are now running in degraded mode. I have run Disk Test from the UI -> System -> Volumes section and it is finished but not sure where to read the results.
Here is the last few logs from this section

Given the 3rd disk does not show any signs of errors and is still not mounting (even if currently removed) should i try to swap/mode disk 4 to slot 3 to see if the problem is actually the slot?
If so and based on this message :
- are the Defrag, Scrub, Balance, Disk Test etc options for the volumes documented somewhere?
- where can i see the Disk Tests results?
- About next steps if i move the disk are there any specific steps to look for?
- Given the test has run can I also consider adding another new disk if the old one still does not work despite being still fully functioning based on SMART test results?
I hope this does not get caught by a filter
