Installing Lime Survey

ReadyNAS 4312 with firmware 6.9.4, HF1
We have the Lime Survey app installed on our two NAS. One of them has a kind of landing page when you click on it but there is a link (which doesn't work) which says "No surveys installed" and on the other NAS I cannot install/set up the app as it looks like I need an SQL instance.
Is this still supported on the ReadyNAS?

To clarify - Netgear never supported it on the ReadyNAS. It's a third-party app that was built by a community member. Netgear distributes it, but with a disclaimer: NOTE: NETGEAR does not warrant any third-party app.
That said, Netgear has been doing more checks with app compatibility in 6.9.x, and it's possible that they did something to disable something that LimeSurvey needs - for instance SQL. Some security fixes might also be interfering with the app's web page.
You might check the log zip file of the NAS - it might give you some more clues on what's going on.
