I attempted to do OS reinstall and Factory Reset from boot menu but it would always have same results as normal boot. Just goes to the login page. We rather not mess with the internal storage on working NAS but I hear what your saying. Do you think some of the files on the internal storage got corrupt during the update to 6x? We needed to update from 4x because of SMB3.

Can you try removing the current disks? Then zero one of them with vendor tools in a windows PC (Lifeguard for Western digital, Seatools for Seagate). Connecting the disk via USB is ok.
Then power up the NAS with only that disk installed. That will automatically do a factory install onto that disk (no boot menu needed), which might clear the problem.

Files on the existing Internal USB:
System Volume Information
csums.md5
initrd.gz
kernal
LDLINUX.SYS
MEMTEST
ROOT.TLZ
SUPPLMNT.TLZ
SYSLINUX.CFG
VERSION.TXT
VPD

That's what it should have All should have recent dates except LDLINUX.SYS and VPD.. Did you try another factory default (if you can even get to the boot menu), or to boot it with just one blank drive, and see what happens? It looks like your flash is right, but it's not installing correctly to the drive(s). But you can try replacing all but VPD from the working one, just in case. If you do that from Windows and it adds a "System Volume Information" file, it won't hurt anything.

Sounds like the factory reset wasn’t done as part of the update process which suggests that there’s nothing wrong with the internal flash.
