Unable to login to my CM2500 modem after activating it with my ISP

Hi there.
I'm trying to login to my CM2500 modem using default password and the password stated in the hardware itself but I'm not able to.
I was able to login before activating it with my ISP and during the activation process but once it was activated, I can no longer login.
Not sure if this has anything to do with it but I do have a small 6 port switch between the modem and the hard wired device in trying with.
I can get to the login page on my devices (computer and cellphone) but unable to login.
No error is shown, just goes back to login page after entering my password.

    Disconnect the ISP coax cable line from the CM modem. Then factory reset the modem. After it's semi ready, can you access it web page at 192.168.100.1 with a wired PC?

    The first connected device downstream from your modem needs to be a router.  The switch does not have any IP addresses to work with without a router upstream providing assignments.
