What version of the PHPMyAdmin add-on?
PRO-PHPMyAdmin_2.5
Have you checked to make sure the mcrypt module is enabled in the PHP add-on configuration (e.g. in Frontview under Add-ons > Installed > PHP5 if this is on your Ultra)? If it's not enabled it, enable it (don't forget to click Apply)
