Duo drops off network after boot

I'd try the on-line support w/o the maintenance agreement, and see what they do. You could also try an OS reinstall first (being careful not to do the factory reset by mistake). This won't hurt your data, but might not cure your problem...
If they require a fee and you have a backup of your data, one alternative is to rebuild the NAS. This will wipe your data and config though. This can be done via the factory reset procedure. Or simply put the disks in a PC and delete the partitions ("volumes") via the disk manager (right-click on computer and choose "manage").
