I have exact same issue after trying to update to 6.2.4
init stuck on
ERR: could not mount boot flash
tried removing all discs and putting unused one in and doing reinstall/resets of course, no luck.
can someone please send me the info?
suspect its something inb the tech section of boot menu but I have never looke dthere.

If you are the original purchaser (and still have warranty left) then contact support. If the flash has failed, then you should get an RMA replacement. The hardware warranty is 3 years.

I have exactly the same issue as everyone here, and the same problem that after trying everything (including USB Recovery to 6.4.2) the error continues. I can't even do a full factory reset, same issue.
I contacted support, who asked me to run RAIDar and advise what I saw, which was same as others: TFTP_REQ. As I could not even factory reset the box, i feel the issue is hardware and am well within my 3 year hardware warranty.
However support advised me without paying for a support call or support contract so I can "work through the issue with them", then there is nothing more they can do - and certainly will not give you an RMA replacement!
So even if it is found to be hardware, your 3 year wty really gets you nothing unless you either also have a support contract or you can categorically prove to them it is definitely hardware, which short of the unit "blowing up" you can't.
Thanks Netgear, it seems I now have a brick unless I pay for support that costs 50% the price of a brand new unit, despite this one being only a year and a bit old.
It would be great if mdgm could also please provide the solution to me as well that earned those Major Kudos, so I can see if it works for me too, as I have tried everything else.

Personally I agree that there should be no support charge for a unit that was bricked migrating to 6.4.x. Though I don't work for Netgear...
