ReadyNAS 3138 - Discard volume(s) - inaccessible volumes help
Hi all,
I had a disk fail {4} and another {2} disconnect afterwards in a four disk X-Raid setup, and seem to have botched the resync.
The data volume is split into data0 {1,2,3,4[missing?]} in Raid 5 and data for the last drive(s?) {3,4} in Raid 1. After resync, the disks show up as fine and are online. They passed a disk test.

Volume: Volume data is Dead.
....
Sep 19, 2025 01:03:29 PM
Disk: Disk Model:ST4000NE001-2MA101 Serial:WS2080XW was removed from Channel 2 of the head unit.
Sep 19, 2025 01:03:28 PM
Disk: Disk in channel 4 (Internal) changed state from ONLINE to FAILED.
Sep 19, 2025 01:03:27 PM
Volume: Volume data health changed from Redundant to Dead.
Jun 23, 2025 11:05:35 PM
System: ReadyNASOS background service started.
I'm hoping I can get some help as local support options look slim.
What would be the recommended first steps to try to diagnose the problem and see if the volumes can be reconstructed to restore access to the filesystem?
Faithfully,
R

All ReadyNAS are end-of-life, so no support from Netgear either.

Did you remove disk 2 at this time?
If not, did something else happen that might have triggered both failures?
If you want me to analyze the logs, then download the full log zip file, and put it into cloud storage. Then send me a link via Private Message (PM). Sent the PM using the envelope icon in the upper right of the forum screen. Make sure the permissions are set so anyone with the link can download.

I did not.
The disk tray seemed to be in place when I got to it, after noticing that all the TimeMachine backups to the NAS were failing.
(This was a few days later.)
A power outage, voltage fluctuation, or temporary hardware failure, is all I can think of.
(Someone servicing the rack could have brushed it by mistake, but that seems extremely unlikely, given the motion needed to pull out the tray. I don't have a tech service ticket that day, but we did have someone quoting us recently.)

I replied to your PM.
A power failure or a hardware glitch could account for it. As I noted in the PM, there are two CMD timeouts (one for each disk) that show up in smart_history.log shortly after the failure.
BTW, I was thinking that perhaps your 3138 was one of the ones included in this service note:
But I checked your NAS serial number, and it is not in the range given in the note.
