I had same issue but closed #20112825 and decided to rebuild from backups. I was already running Radiator 4.2.22 and had successfully upgraded 4 x 1TB to 4 x 2TB the system was working without problem for days. I then had the same issue when the scheduled File System Consistency Check failed. I still had my original 1TB drives and since they were only a few days old i decided to start again.
Big job though. I'll be hesitant to change disks and expand again after what appeared to be a simple job completed and running happily, very quickly went to complete data loss. You wonder how the file system fails when the upgrade process appeared to run many checks throughout the process?
