It sounds like the previous USB recovery worked but, for unknown reasons, the unit did not shut down to let you know it was successful.

Yes, that's how I was able to create the USB recovery drive and boot to ReadyNAS using that recovery drive. Are you thinking the USB recovery drive is somehow corrupt?

I should have mentioned I don't have a Windows PC, so I used a Windows 10 virtual machine running in VMWare Fusion to create the USB recovery disk.

There are different guides for legacy NAS, and I wanted to confirm that you used the one for OS-6.
Also, the NAS can be fussy about the USB drive used. Generally older and smaller thumb drives work better. So you might want to try a different drive if you have one.
But I haven't seen that particular error before - hopefully JohnCM_S or Marc_V will chime in.

Well, it working again, but I'm not sure why. I got a different USB drive and set it up with recovery software. I tried to boot from it, but as I held the OK button, the OK button light went off and the display said "booting..." rather then "booting from recovery drive...". The same error got displayed on the monitor, but the boot process continued this time. I removed the USB drive and rebooted and it succesfully booted. Not sure why, but it does have me thinking something is not healthy with the device.
