Talked with Netgear support. Very helpful. I referred them to this thread for background. They suggested a full reinstall of the firmware. The only settings lost (back to factory defaults) were IP and FV admin login. This definitely made a performance improvement but it is still nowhere near as fast as it has been for the entire rest of its life.
I also testing accessing the NAS through a virtual Windows 7 machine on one of my Macs (one that is slow) the speeds there seemed better (although I was only able to test that through WiFi so I did not bother doing extensive testing).
I am leaning toward a Mac/AFP issue and perhaps with 4.2.23.
