CM700 Modem keeps dropping and speeds are super slow

CM700 Modem keeps dropping and speeds are super slow
As of the begining of this week my cable modem keeps dropping service and is having horrible speed problem. I have COX with the 300mbps plan and I am barely pulling 30mbps on a good day. I have looked at the modem logs and noticed a lot of timeouts and partial service. Along with horrible power levels on the upstream. Please help advise if its the modem or provider. Here is my log and connection results.

Have the ISP check the signal and line quality UP to the modem.
Be sure there are not coax cable line splitters in the between the modem and ISP service box.
Be sure your using good quality RG6 coax cable up to the modem.
Be sure to test speeds directly connected to the modem with a wired LAN cable PC. Test with a different PC to compare results.

do what @FURRYe38 recommended.
check your lines/connectors/splitters/amplifiers. Your downstream channels 13-22 are way out of spec on power and its giving you a bunch of errors. You should be ideally between -7dbmv and +7dbmv. Those channels are more than -20dbmv.
so check the coax line yourself within your home and if it stays the same, your isp needs to check your lines.
