Problem with new modem CM1200

My modem is CM1200. My ISP is Suddenlink. Two speeds with highest being 5K.

Router is Nighthawk AC1900. Modem constantly going down. Did i set it wrong?

Included file has connection and event log.

    You don't have errors in your cable connections page but your power levels are a bit interesting. You should only have around 3.5dbmv between channels and you're going from a -4.3dbmv to a +2.9. roughly a 7.2dbmv difference. not horrible but not great. 

    and you're logs are a bit all over the place. 

    I'd check your line for issues with splitters, amplifiers, or attenuators. remove them if you have them. 

    check for damage to the line like kinks, loose/old/cheap/bad connectors.

      Apparently, there is an Amplifier as shown by the photos. But Im not sure where all the outputs go to. I know one goes to the telephone included in the service. Should I buy a new amplifier and just transfer all the cables to the new amplifier??

      Cant upload the photos. The amplifier is Extreme Broadband, Infinity Premise Amplifier Model IPA1008D-RSVF. 8 outputs, 1 input, one -3.5db VolP/Modem (which goes to my modem).

        You can add them using the "choose file option". If they're to large, they might not. 

        If you direct connect the modem without the amplifier, so just bypass it temporarily, does it work?
