Per Netgear support, pass along to help anyone else out:
Here are the steps that you should try to reset the password :
FSM726v2 (reset procedure)
1. While booting up, press to enter boot code menu.
2. Press in boot code main menu to enter debug menu.
3. Press "5" to choose "Flash Access".
4. Press "2" to choose "Flash Erase"
5."Start of sector #": Press 23 Enter
6."End of sector #": Press 31 Enter
7. Confirm this command.
FSM726v1
You need to interrupt the boot sequence and use a hideen system command:
1) Power on switch, press"ESC" key very quickly to go to Mini Boot Monitor mode. (If you press too late you will go into the option to boot from Flash or Net via TFTP.)
2) Press "Enter" key.
3) At the commend prompt Type: . EmergencyPasswordReset (Exactly as written it is case sensitive and doesn''t like spaces. This is a hideen command.)
4) There is then a question . "Are you sure? (y/n)". The answer is "X". This a capital X - It will disable password.
5) You will get a response that the system password has been disabled - if you don''t get this confirmation it hasn''t worked because you have mistyped something but it won''t tell you that.)
5) Enter "g" to re boot. .g
This will disable the password for switch on reboot.
