CM500 - Upstream channels not locking
Just in the last few days have noticed random - but severe - amounts of lag while gaming on a PS4 Pro. Noticed that a light was amber on my modem. Reset the modem and it went away and everything was fine until the next day. Similar things have happened over a couple days. Now the light won't go away. According to the manual (and looking at the modem connections) the light is due to an issue with my upstream channels. 3 of the 4 upstream channels are not locked and have "0" for values.
I live in an apartment by myself. Nothing has been touched/moved since these issues started happening. I've tried restarting modem and router multiple times. I just got off the phone with my ISP (xfinity) and they didn't even attempt to help me. They said since I didn't have one of their modems they could give me the phone number for netgear's support. I specifically asked if this could be an ISP problem and was told "no".
Would this be causing my lag I've been experiencing? Is this actually an ISP problem and was just given the runaround since I don't rent one of their modems? Is there anything I can do to fix this issue or do I need a new modem? This one is ~5 years old.
I have taken screen shots of the down/up channels and event log. Hopefully this link works, if the files are hidden, I can provide another link.
Thanks for any assistance.

Forgot to mention download and upload speeds seem to be ok. Google speed test shows me at ~450 down and ~17 up.

You upstream power levels are boarderline high.
and t3/t4 errors tend to be line issues. Not modem issues.
If you have any splitters, amplifiers, attenuators ran in the coax line, remove them.

So today all the lights are green on the modem, and all upstream channels are locked...
I didn't change anything. I came out to look at the router this morning and it was all green. Checked the modem and found all upstream channels are locked.
There is a splitter to my cable tv box and that's it.

Would be on the ISP side of things they may have changed something on the line.
