Disk Test Failure

The SMART extended self-test failed. The most likely cause is there's one or more bad sectors on the disk, but that sector hasn't been accessed during normal usage. Since all the other disks passed, you should be safe to run a scrub, which should expose the error and correct it automatically.

A failed SMART extended self-test is the only thing that will prompt a message like "Disk test failed on disk in channel 2, model XXXXXXXXXXX, serial XXXXXXXXXX". Your logs also show that the last SMART self-test failed.
