3 copies in different places is my threshold also. I've lost data (pre-nas) when I found I couldn't read older files backed up on a USB drive. Of course you don't find out that the backup is bad until the primary fails... Since I've raised the bar to 3 copies I haven't lost any data, though I have had some device failures.

My Duo lost everything after a power failure a while back, and I had to rebuild the array from scratch (and also bought my first UPS, which is another lesson learned). I've also had some individual disk failures in my Pro and my PCs. But so far, at least one copy has survived intact.
