Factory default?? As in wipe everything? Did you have data on the NAS?
If you installed something and the NAS no longer boots, and it can be what you installed that's preventing the system from booting, or a full root. But if you have the same issue after a factory default, there is something else.
What usb restore procedure did you follow? Where did you find it? I believe there are procedures for RAIDiator 4.1 and ReadyNAS OS 6, but an Ultra 2 runs RAIDiator 4.2, which is very different. I don't recall seeing one for RAIDiator 4.2.
Did you do os reinstall and factory default before or after usb boot recovery?
(This usb boot recovery is rarely necessary and it seems a lot of people attempt it in desperation. Sometimes making things worse, cf Note/Warning section in the kb article. It's a bit like "my laptop doesn't boot in windows so i flashed the bios".)

yes, I accessed the boot menu using the reset button on the back and then the LED-indicators on the front to do both the Factory Default and also the OS reinstall. Tried everything to get it back to "standard" settings.

I did not have anything important on the disks, I took them out, made a disk check in my pc and formatted both.
I think I used this instruction for the USB restore, but do not think it ever worked:
I just want to make the readynas boot again, its of no use right now :-(
