I guess I am still a bit confused on what you've done.

I am thinking that you have
(a) created a share, and gave it HTTP/S access
(b) Put php pages into the share
(c) are navigating from the browser to the php page in the share
You get the logon when you first access your php page, but are not seeing a logout.
And you would like to know what code to add to your web page to create the logout.
Is all the above correct?
