"modem" (modem-only) or modem+router?

If that means that you have cascaded two routers, then that could
explain multiple problems, including this one. To which device is the
PS4 connected?
Why do you have the second router (WNDR4500v3)? Generally, you want
one router, which means that either the C6250 should be in modem-only
("bridge") mode, or else the WNDR4500v3 should be in
wireless-access-point mode. If the router functions of the C6250
satisfy your router needs, and you want to expand your wireless-network
coverage, then configure the WNDR4500v3 as a wireless access point.
Visit http://netgear.com/support , put in your model number, and look
for Documentation. Get the User Manual. Look for "Use the Router as a
Wireless Access Point".
