Hi marioalpha,
This is the link where I got the steps that I have summarized for you recently. However, let me share this old forum thread I found and it might help. Take note that some of the links on the old forum thread I have shared are not active anymore. Possible reason here might be that the FSM726 is an old model of a managed switch.

Hi marioalpha,
Welcome to the community! :smileyhappy:
It seems that you might need to reset the CLI password. Here are the steps below to reset the password:
a. While booting up, press to enter boot code menu.
b. Press in boot code main menu to enter debug menu.
c. Press "5" to choose "Flash Access."
d. Press "2" to choose "Flash Erase."
e. "Start of sector #": Press 23 Enter.
f. "End of sector #": Press 31 Enter.
g. Confirm this command.
Hope it helps.
