I'm very happy to find a topic referring exactly the issue i faced.
I also received a replacement switch put problem was still the same (last firmware from the website, etc ...).
After hours of troubleshooting, i found out that it's related to the cascaded switch behind this one.
This GS728TP was connected on a GS724TP, and both are POE switches, i disabled the POE feature on both switches on the ports used to cascade them and no issues anymore.
I don't know if you had same setup but in my case it's now solved ! :)

I am having the same exact issues. I purchased 2 new GS728TP's. Configured them and installed them the same way. One is working great, the other stop supplying POE after a couple of days. I rebooted it and everything worked fine. A couple days later it stop supplying POE again. I opened a service request and after a lot of troubleshooting I was told to go back to my supplier and have them send me a new one. I did this, and now the replacement is doing the same thing. Supplys POE for a while after a reboot, but then stops. I am only pulling 50W total with no one device higher than 6.6W. Both switches are at Software Version 6.0.0.64. I am stuck on what to do. Any help out there?
