If this is the only problem with the PS5, would be something to contact Sony support about regarding there services.
What FW version is loaded on the modem?
Please post a copy and paste of the modems connection status and event log page.

Only one critical and not lots.
Power levels look ok from what I can tell. Messy post. 🙄
I think your seeing some correctables and un-correctables in those columns that should be mostly zeros.
Be sure there are no coax cable line splitters in the between the modem and ISP service box.
Be sure your using good quality RG6 coax cable up to the modem.
Start with removing any amplifiers, signal attenuators, or splitters from the coax.
From there check the line for kinks, damage, moisture in the line.
Check the connectors for improperly made ends, foil touching the copper coax line, loose connections, bad/old/cheap connectors, or corroded connections. Replace them if you do.
Be sure to power OFF the modem for 1 minute then back ON.

Any Criticals, Errors or Warnings seen in the event logs needs to be reviewed and resolved by the ISP. Indicates a signal issue on the ISP line up to the modem.
Make contact with Sony regarding what your seeing on the game console.

I did swap out the coax (old one looked just fine though) and it did not resolve the problem.
I did some random digging and learned that I don't need ports forwarded with upnp enabled, if working correctly (I think ). So I turned of upnp and kept the ports forwarded, no change. Then I enabled upnp and deleted all the forwarded ports, still no change and issue persisted.
I tried to use the multi gig port assuming I'd get slightly faster connection with my 1.2gbps internet, but that port had been the worse by far. No thumbnails would load in on the console at all, with the regular ethernet ports I still get some thumbnails. Also read that this port may be well be defective 🤷🏽, in many cases
Finally, last resort i just went through every setting i could find in my router page, and came across ethernet ports aggregation. Read up on it and thought i really shouldn't need this, but I enabled it anyways, let the modem do is thing and reset. Been working flawlessly since!!! All thumbnails load instantly, everything feels snappy on console interface again, nat type is correct and open on the games I need. Pretty stoked haha. It's only been couple hours so I'll have to see if this holds up.

Edit: reset ps5 connection to auto settings on lan settings, no more static ip. Did this first and there was no change. It's currently still auto but working fine with aggregate

Hmmm, interesteing LAG seems to solve that though could be red herring or masking the real issue. Not sure how that would effect how the game console is connected. LAG feature would only be needed for LAG supporting devices. Otherwise should be disabled.
Yes, uPnP if enabled, PF configurations should not be configured.
Generally for most gaming, Sony and MS, no manual PF configurations are needed. Just uPnP handles everything.
Something to try later on, Factory reset and setup from scratch. Don't enabled anything, LAG or set any PF configurations. Don't enable Armor, Acess Controls, IPv6 or Traffic Meter. Leave uPnP default enabled. NAT should be good and connection should be good as well.

Think you were correct about the red herring haha. Issue came back soon enough. I had contacted comcast earlier and was directed to automated trouble shooting which stated all was good on their end. Decided to try again and speak to agent this time, he didn't really offer me any real solution, but he increased my speed from 1.2 to 2000Mbps and reduced my monthly bill lmao. But he did say he sent over new advanced signals and my router did reset, and he stated all connections were healthy and good. I did also take your advice and do full factory reset beforehand, I did have to disable the aggregate even after the reset, but all else was correct default. Didn't open ports nor mess with any other settings except my wifi channel names and pw. It didn't help sadly, issue persisted. As of right now the router is off a fresh reset from comcast so it'll likely work for a few hours lol. The agent stated whatever he did should resolve the issue.. if it doesn't, would it be fair to assume the router itself is at fault/defective?

Do you have firware protection enabled by default?
Maybe try below configurations:
1) Log in to webGUI.
2) Click "ADVENCED" -> "Setup" -> "WAN Setup"
3) Click "Disable IPv4 Firewall Protection" and "Disable Port Scan and DoS Protection" checkbox.

Hi, did you mean that you already tried disabling firewall protection/DDOS and the issue is still there?
Two more questions to clarify-
1. Does the issue happen since device was upgraded to v5.1.1.8? or it exists in previous version? v2.x?
2. Only PS5 has this issue or other LAN devices exhibit this issue as well?

Firmware 5118 was working fine without issues for a long time(over year). Not sure if i ever even ran the older firmware tbh.
I have another console on lan, nintendo switch. It seems to be working fine. When I plug in my laptop via ethernet that works fine as well.
I did have an older ps on which this issue arose after a year of no issues, but I got another ps since then and this problem persisted from day one on that unit. I have tried
I'll report back with an update, it takes some time for issue to arise sometimes (anywhere from 5 mins- few hours)

1. Disable ipv6, which i don't think i can do with my isp nor cax80
2. OP used a second router to connect to just ps5. In that router he can disable ipv6, guess that's his work around.
Nobody else had any real solution aside from just resetting or resetting cable over n over a the issue keeps popping up ...

Sounds like this maybe a Sony issue rather than CAX80 issue.
Only other way to help narrow that down would be to put a ISP owned gateway online and check it with the PS online and see if same thing happens.
v5 FW has only been out for about two months.
Was this issue happening before you put v% FW on the CAX80 or did it start afterwards?
