SMART self-assessment test

SMART self-assessment test
Hi,
My fourth disk did fail, so I did change it for a new and fresh one. But since the change I have started to get nightly event that the disk4 (which was swapped) does not pass the SMART self-assessment test. The disk is working and the X-RAID is complete and there is no problem in the system, only this event that now occurs on regular basis (I guess the SMART check is done every night at 0400).
The event;
"Disk 4 did not pass SMART self-assessment test. Please replace this disk as soon as possible. Growing SMART errors indicate a disk that may fail soon. If the errors continue to increase, you should be prepared to replace the disk."
When checking the SMART info, there is nothing that would indicate any issues with this disk, so I don't know really if the SMART test is correct, or if the disk is really failing.
Any clues what could go on, or is it better to just change the disk?
Thinking that the NAS HW starting to be well over 5-6 years old (don't even remember when I did buy it), maybe there could be something else than the disks? it has been running 24/7 since I got it. (not with the same disks of course)

A reallocated sector count of ~4000 is very bad. The drive needs to be replaced.
